Marie-Claire Chartrand
Lead Senior Care Consultant | MTS, RSW, Cert EM
Marie-Claire Chartrand, our lead senior care consultant bases her practice on her 30-year career with Nova Scotia’s Continuing Care Program. She has unparalleled experience helping clients navigate the private and public health care systems. Marie-Claire is a Licensed Social Worker and has worked across the healthcare spectrum, in hospitals and the community, with both palliative and geriatric clients and their families. She has a Social Development Studies degree from the University of Waterloo and a Master's degree in Theological Studies from the Atlantic School of Theology. Marie-Claire is a Designated Capacity Assessor under the Adult Capacity and Decision-Making Act since 2019.
Nova Scotia’s only Elder Mediator, certified by Family Mediation Canada, Marie-Claire is skilled at helping clients manage complex family relationships to achieve their goal of helping loved ones. Marie-Claire provides services in both English and French.
Brenda MacIsaac
Senior Care Consultant | RN, BScN
Brenda MacIsaac graduated from the PEI School of Nursing before completing a Bachelor of Science degree in Nursing at Saint Francis Xavier University. She later obtained national certification in cardiovascular nursing. Brenda has a varied 41-year career in nursing. She worked on medical wards, cardiac care units and as a nurse educator before entering the field of Continuing Care, where she has been assessing and meeting client needs for the past 25 years. She has wide experience in hospital discharge planning, palliative care, long-term care, home care, dementia care, and in supporting families facing complicated challenges.
Wendy Wierstra
Senior Care Consultant | BA, BSW, RSW
Wendy Wierstra is a Registered Social Worker who has practiced in the field of Geriatrics for 25 years. During her career, Wendy has worked in both facility and community settings. Wendy graduated from Saint Mary's University with a Bachelor of Arts in Sociology and completed a Bachelor of Social Work from the Dalhousie University School of Social Work. Wendy later further specialized her education with a Certificate in Aging from Hunter College in New York.
For the last 20 years, Wendy has worked in the Continuing Care system. In this role, she has assisted many individuals and families, helping them find the unique balance of support to manage their lifestyle and care. Wendy is skilled in navigating public and private homecare systems. Wendy has extensive experience in long term care, dementia care, advance care planning and in the provision of caregiver support both in individual and group settings.
